Acute idiopathic sensory polyneuropathy can present with ocular findings. We describe a patient with acute Charcot-Marie-Tooth disease (CMT) type 1 who presented with visual and ocular findings without peripheral neuropathy. The visual loss was initially thought to be optic neuritis due to a right inferior temporal, white, subretinal mass. We present the case of a patient with an acute polyneuropathy and an ocular finding of a right inferotemporal, white, subretinal mass. Magnetic resonance imaging showed a mass lesion in the right inferior temporal quadrant with corresponding hyperintense signal on T2-weighted and fluid-attenuated inversion recovery images. No magnetic resonance imaging or optical coherence tomography abnormalities were seen in the periphery or optic nerve. An ophthalmologic examination revealed right optic disc edema, nonprogressive visual loss, and a right inferotemporal, white, subretinal lesion. Despite extensive investigation, no causal relationship between the optic disc edema and the subretinal lesion could be determined. The subretinal lesion was excised and histopathologic examination showed a lipid-rich macrophage. Ocular findings are not uncommon in CMT and may be related to a few different pathological mechanisms. A major differential diagnosis is acute idiopathic sensory polyneuropathy, a heterogeneous condition including a variety of acute neurological conditions that can cause visual loss. Although visual symptoms may resolve in CMT, they can result from other etiologies. The diagnosis of an acute visual loss in the setting of a peripheral neuropathy can be challenging.using Newtonsoft.Json;
